People living with Type 1 or Type 2 Diabetes can access an Enhanced Primary Care Plan that allows up to 5 x Medicare funded visits to any Allied Health Professional within the calendar year. Simply visit your Doctor and ask about this at your next visit.

People living with Type 2 Diabetes can access another Medicare-funded program that allows them to have one initial assessment with an Exercise Physiologist, and then 8 x fully-funded group exercise sessions to start you on an exercise plan!

We will get started right away on assessing your current health status and figure out an individually-tailored plan to improve diabetes control and reduce the risk of other health conditions.

Your Exercise Physiologist will help you set goals, such as maintaining healthy blood-glucose levels, improving diet control and improving weight management.

They will set up a physical activity plan to achieve these goals. It may include home exercises, diabetes exercise classes, dietary advice and more.
